- You were recently diagnosed with type 2 diabetes or prediabetes
- You’ve lived with diabetes for a while and want more support
- Your doctor recommended lifestyle changes and you’re not sure where to start
How Does It Work?
Initial visit (about 30 minutes): we get to know you: your health, your habits, your challenges and what matters to you. Together, we set your first goals.
Follow-ups (about 15 minutes): we meet regularly to check in, celebrate your progress and adjust the plan as needed.

No referral needed. Detailed receipt provided for your private insurance.
Why Clinique 360?
Our coaching is provided by Melissa Peritz, clinical nurse, who follows Diabetes Canada’s guidelines. She works in collaboration with our medical team and will refer you to your doctor for any questions about your treatment.

How much does a diabetes nurse consultation cost?
The initial visit is $85 and each follow-up is $45.
Is it covered by RAMQ?
No. This is a private pay service.
Will my insurance reimburse it?
Many private insurance plans cover nursing services. Check with your insurer. We’ll provide a detailed receipt.
Do I need a referral?
No. You can book directly.
Does this replace my medical follow-up?
No. This service complements your doctor’s care by giving you time, a listening ear and practical tools between appointments.
How many sessions will I need?
It depends on your needs and goals. We’ll discuss it together at your first visit.
Can sessions be done virtually?
Yes. Follow-ups can be done virtually or in person, as you prefer. The initial visit takes place at our Pointe-Claire clinic.
What should I bring to my first visit?
Your recent lab results, your list of medications and your glucose meter or CGM reader, if you have one.
